Surrender Application and Agreement

    I hereby voluntarily and irrevocably give, donate, surrender and release to We Love Animals Rescue, hereinafter referred to as WLAR, the following animal.
    I represent and warrant that I am the lawful adult owner of the Animal, and I have full power and authority to surrender the Animal to WLAR. No other person has any legal or equitable ownership interest in the Animal. I have disclosed to WLAR all material information regarding the medical and behavioral history of the Animal. I willfully surrender all medical records and information pertaining to the Animal. WLAR has my permission to contact my veterinarian for any necessary information pertaining to my Animal, and I hereby consent to the release of all medical information by any medical provider.
    I further acknowledge that I am releasing the Animal completely voluntarily and that no representations, considerations or promises of any kind have been made to me by WLAR or any of its representatives.
    1. Ownership & TransferI, the undersigned, certify that I am the lawful owner or authorized guardian of the above-described animal (the “Animal”) and have the right to surrender or assign care of the Animal to We Love Animals Rescue, Inc. (WLAR). Upon acceptance, I transfer all rights, title, and interest in the Animal to WLAR, relinquishing any ownership or claims.
    2. Health & History Disclosure
    I agree to provide all known information regarding the Animal’s medical history, behavior, temperament, bite history, and any past aggression or ongoing medical conditions. I understand WLAR is relying on my disclosures. I acknowledge that WLAR accepts the Animal as is, with no warranty or guarantee of future health or behavior.
    3. Animal Condition & Care
    Upon surrender, WLAR assumes custody and care of the Animal. I understand WLAR may place the Animal in foster care, a shelter facility, or arrange for veterinary treatment, and I relinquish all control over those decisions after surrender.
    4. No Rehoming or Return Rights
    I understand that surrendering this Animal is permanent and final. I waive any right to reclaim the Animal, unless specifically agreed otherwise in writing by WLAR. WLAR may place, rehabilitate, or rehome the Animal at its sole discretion.
    5. Assumption of Risk
    I acknowledge that surrendering an animal involves risks associated with health, disease, temperament assessments, or unforeseen medical problems. I accept these risks and release WLAR from any liability relating to preexisting conditions or unknown issues.
    6. Hold Harmless / Indemnification
    To the fullest extent permitted by applicable law, I agree to defend, indemnify, and hold harmless We Love Animals Rescue, Inc., its officers, directors, employees, agents, volunteers, successors, and assigns (the “Indemnitees”) from and against any and all claims, suits, demands, damages, liabilities, losses, costs, or expenses (including attorney’s fees) arising out of or related in any way to: Any misrepresentation or omission I have made regarding the Animal’s health, behavior, or medical history; Any claim, injury, damage or liability relating to the Animal’s past conduct or conditions, whether known or unknown to me; Any breach by me of this surrender agreement; Any negligent or intentional act or omission by me in connection with the surrender or transfer of the Animal. This indemnification and hold harmless obligation shall survive this agreement and remain in effect indefinitely, binding my heirs, executors, and assigns.
    7. Acknowledgment & Signature
    I hereby affirm that I have read, understand, and voluntarily agree to all terms in this Surrender / Intake Agreement. I certify that all information I provided is true, complete, and accurate to the best of my knowledge. I understand that surrendering this Animal is a permanent decision as described above.
