1. Adoption Terms
I, the undersigned, agree to provide the adopted animal (the “Animal”) with proper and humane care, including food, water, shelter, exercise, and necessary veterinary attention. I understand that this adoption is permanent and that the Animal may not be transferred, sold, abandoned, or surrendered to another party or shelter without the express written consent of

2. Health and Behavior Disclosure
I acknowledge that WLAR has provided all available information regarding the Animal’s known health and behavioral history. I understand that WLAR cannot guarantee the Animal’s temperament, long-term health, or behavior, and that unforeseen issues may arise after adoption.
3. Veterinary Care
I agree to have the Animal examined by a licensed veterinarian within 14 days of adoption and to provide all necessary vaccinations, preventative care, and medical treatment as recommended by a veterinarian.
4. Return Policy
If, for any reason, I can no longer care for the Animal, I agree to contact WLAR immediately to discuss return options. WLAR reserves the right to accept or decline the return based on available space and resources.
5. Hold Harmless / Indemnification Agreement

, its officers, directors, volunteers, agents, representatives, successors, and assigns (collectively, the “Indemnitees”) from and against any and all claims, demands, damages, injuries, losses, liabilities, costs, or expenses (including attorney’s fees) arising from or related in any way to:
My ownership, care, custody, or control of the Animal after adoption;
Any damage or injury (including death) to persons, animals, or property caused by the Animal;
Any breach by me of this agreement; and
Any negligent or intentional acts or omissions by me or anyone residing with me.
I understand and agree that this release applies even if any claim arises out of negligence on the part of WLAR or its representatives. This agreement shall remain in full force and effect for the lifetime of the Animal and shall be binding upon my heirs, executors, and assigns.
6. Acknowledgment
I have read, understood, and voluntarily signed this Adoption Agreement. I certify that all information provided in my application and this agreement is accurate and complete.
